URL Financial Online Licensing Appointment Process
It’s fast, easy and efficient!
Items you will need before you begin the online appointment process:

Copy of your E&O (if you are requesting carriers that require you to have E&O coverage)

Copy of your life and health (and/or long term care) license

Copy of voided check and bank information for Direct Deposit requests
Click on the hyperlink “Begin Online Contracting”. You will be asked for a User ID and a password. Click on the hyperlink “AGENT REGISTRATION” and you will be instructed on how to create a secure ID, password and 4 digit pin number. Once created, you will need this information to immediately sign back in to the licensing system.
Once you’ve signed in, the first page will give you an overview of the Efficient Forms process you are about to begin. Page 2 will allow you to choose the carriers you wish to have appointments with.
So that we can process your licensing in the most efficient manner possible we will gladly contract you with a maximum of 3 carriers initially. If you need to be contracted for more than 3 carriers before your production with URL justifies this you will need prior approval from a division vice president. Please call 1.800.926.8875 and speak with us.
* Helpful hint! You need only put information in the areas that are marked by the small red asterisk. This will save you additional time in completing the process.
The system will allow you to tailor your request to your specific needs, whether it is an individual or corporate appointment request, with or without commission assignments. You will also be given the option for Direct Deposit. Using Direct Deposit is a great way to receive your commissions in the most time efficient manner and some carriers (like West Coast Life) require you to use direct deposit.
As you near the end of the online process, you’ll receive a message page indicating that you are almost finished. As you finish the last pages, you’ll be asked to give the 4 digit pin number you chose earlier and to submit your carrier appointment requests. Once you’ve submitted these requests, your final screen page allows you to print cover sheets that are necessary to finalize the process.
Print out the cover sheets and attach a copy of your license, E&O and signature as requested by each individual form to the toll free number indicated on the cover sheets. This will allow your information to be directly scanned into the system and trigger our licensing department that you’ve requested a carrier appointment. And for your convenience, in the future if you find you need an additional carrier appointment, simply sign back in to the licensing system (using the ID information you created earlier), and click on “request additional carriers”. Your information will automatically re-populate the system pages.
Remember, many carriers require pre-appointment before taking application, so if you are unsure if your appointment is complete, or if pre-appointment is necessary, check with your Brokerage Manager before taking application.
